W.R. Grace’s Voluntary Libby Medical Program
This is a voluntary medical plan and although acceptance may provide some medical and prescription benefits for treatment of your asbestos related disease condition, it does not guarantee that everything your medical provider orders for your condition will be covered. Coverage of services (doctor visits, x-rays, PFTs, chest CT scans, supplemental oxygen, hospitalizations, surgeries, etc.) will only be covered if HNA physicians and radiologists agree that it is related to your asbestos related disease condition. They do not base these decisions on the opinion of the treating medical providers. Thus, it is recommended that individuals who are accepted on this plan also secure LAMP Supplemental Benefits which may cover some services that this plan denies.

If you are diagnosed with an asbestos related disease from a medical provider, you are a candidate for the Grace Libby Medical Program administered by Health Network of America (HNA)/Triveris. You can apply to the program but you must have assistance from your medical provider. However, acceptance is not guaranteed based on your medical provider's recommendation. Acceptance is based on the opinion of physicians and radiologists (x-ray readers) selected by the Grace Libby Medical Program.

The HNA application needs to be completed by the individual and the medical provider who made the diagnosis of an asbestos related disease. Then the diagnosing medical provider's office can submit the application to HNA along with your medical records (including office notes, pulmonary function test reports (PFT/breathing tests), radiology reports, and information so HNA will be able to get copies of the actual radiographic images (x-rays and CTs). Grace Libby Medical Plan applications are available from HNA, CARD, or the EPA information center in Libby. It could take up to three months to determine if you will be accepted on the plan. If it takes longer than three months to find out if you have been accepted, it is recommended that you call HNA and check on your application status.
